TMJ JAW PAIN, & TMJ DISORDER TREATMENT

One of the causes of  TMJ pain and discomfort is missing teeth not restored,causing  domino effect on the bite, gums , spacing and gaps of  the teeth,and finally the tmj structure.The tmj disorder treatment starts with finding the etiologic factor causing the problem,and then eliminating the problem.
 This Patient had tmj symptoms or TMD.
The reason was two folds:
First due to tooth loss in the back ,causing uneven forces on the bite ,when patient eating.,and
the second reason was due to CLENCHING teeth and BRUXING of  his teeth.,  notice the flaring of the teeth,( in the before picture),  and the
spacing and gaps on his teeth ,causing teeth movement, and un balanced biting forces on the rest of patients teeth, causing bite trauma (occlusal trauma),  tmj discomfort (tmj pain), and tmd.
Due to missing teeth ,and cosmetic smile wishes of the patient,the treatment consisted of FULL MOUTH RESTORATION (full mouth reconstruction)
using combination of PORCELAIN VENEERS,DENTAL TOOTH IMPLANT,implant dentistry in the back teeth, PORCELAIN CROWNS and
PORCELAIN BRIDGES.,all followed by a NIGHT GUARD to avoid fractures of the teeth or porcelain ,and maintaining the FULL MOUTH RESTORATION.

Besides the width of the tooth being %75 to %80 of the length,we also have a measurement generally to find out, what the length of the upper front tooth should be. How ? Because of this point mentioned ,i generally do not recommend having 6 teeth veneered ,or 6 (dental veneers )done. Your canine teeth already are bigger and darker than the rest of your teeth,if the patient changes the color to a more whiter desired color ,those 6 teeth will stick out like a sore thumb,and hides the rest of teeth, specially when you are looking at the smile from the side (the back teeth will look much smaller).

Patients interested in porcelain veneers often wonder how white the veneers should be.

Teeth are not monochromatic, so using a variety of shades yields a natural look, said Dr. Anthony Mobasser, a Los Angeles dentist who specializes in cosmetic and reconstructive dentistry.

Some Background

First let’s address the obvious question and define porcelain veneers.

A porcelain veneer is a thin sheet of porcelain that can be affixed to the surface of a tooth to improve smiles by covering chipped, broken, stained or misshaped teeth. Porcelain is usually thought of as a delicate material, but when bonded to a tooth, it is incredibly strong and durable.

Selecting a Veneer Shade

Mobasser recommends patients undergo a teeth whitening treatment to whiten their natural teeth prior to placing porcelain veneers. Because veneers are extremely thin, a tooth that is dark or deeply stained could provide a shading effect once the veneers are affixed that causes the finished product to be darker than desired.

Why not choose a thicker porcelain veneer?

“The translucency of porcelain veneers gives them the ability to mimic natural tooth enamel and creates a natural appearance,” Mobasser said.

Using more than one color in making the veneers also provides variation and avoids teeth looking fake or artificial. Eye teeth tend to be darker than the others, while front teeth typically are the whitest. Molars most often are a shade between the two.

Another important factor to consider is the surface texture of porcelain veneers, Mobasser said. Adequate texture helps to break up light reflections and make the veneer look more natural.

The most often heard about, however, is the Vita shade guide. This guide divides tooth color into four basic shade ranges:

A (reddish brown)
B (reddish yellow)

In the A range there are five levels of darkness. Ranges B, C and D each have four levels.

“When I evaluate a patient for porcelain veneers, I am concerned with getting the optimum results,” Mobasser said. “In addition to considering the color of the patient’s natural teeth, I also evaluate their complexion, hair color, and to some extent, their eye color. It is important to me that a feature as dominant as a smile compliments all of their other facial features.”
